What to Expect

So what can you expect when you walk through our door for the first time? You’ll receive a warm greeting from our friendly team, and we’ll ask you for your driver’s license or other form of ID, along with a valid dental insurance card.

Next, we’ll give you 3 forms to fill out: a health history form, a payment form, and a consent form that gives Dr. Leesa and her team permission to perform the necessary services for your treatment.
